I was a member of a plenary on incident recording as a part of a continuous quality improvement (CQI) program. Other members of the plenary included two pharmacists, who have been using incident recording for many years and who are committed to patient safety in their practice. With our input, the College of Alberta was given the instruction to begin research on how to integrate this CQI program into their pharmacies.
